Dr. Jason Emer uses Alloclae, a permanent fat-cell filler, to add girth and smooth irregularities from prior treatments, then applies microdroplet silicone to the glans to build head size. The procedure is done in-office with nerve blocks, takes under an hour, and requires one to two weeks of avoiding sexual activity while bruising resolves. Individual results vary.
What Alloclae does in this area
Alloclae is a permanent filler made of adipocyte cells (fat cells) and extracellular matrix. The ECM signals your body to produce its own fat, which means the volume you see is partly from the product and partly from your own tissue response. Dr. Emer uses it here to add girth, smooth out irregularities from previous filler treatments, and create a more natural feel than hyaluronic acid or PMMA alone.
He injects it with a cannula, the same tool he uses in surgery. The cannula lets him work through scar tissue from prior injections, breaking apart nodules and evening out areas where collagen has built unevenly. One patient in this video had Bellafill over the years, and you could see small lumps near the circumcision line and under the corona. The Alloclae camouflaged those irregularities and added width at the same time.
The result is permanent as long as you do not lose significant weight. Dr. Emer has not seen patients lose penile volume during weight loss, but the principle is the same as fat grafting elsewhere: the fat cells that survive are there for good. Alloclae has been used on calves, chest, face, shoulders and buttocks with the same permanence.
Silicone for the glans
The glans (head) is spongy tissue. Hyaluronic acid filler absorbs into that sponge without much visible expansion, so you need large volumes and the result does not last. Dr. Emer uses FDA-approved silicone oil instead, injected in microdroplets just under the surface of the skin, almost like treating a fine wrinkle. The silicone stays where it is placed and does not migrate when injected this way.
He limits each session to under two to two and a half cc's. Studies on silicone for acne scars showed no complications when the dose stayed below two cc's per session. Staying in that range means your body does not react against the material. You build size over two to four sessions, not all at once.
You will see some expansion immediately, and more as collagen forms around the silicone over the following weeks. Some men see a pebbly texture at first, which softens with time. Shockwave therapy at home helps the tissue remodel. The corona (the ridge) and the body of the glans both respond, and the final size depends on how much collagen your body builds. Individual results vary.
Breaking through scar tissue from prior work
If you have had Bellafill, hyaluronic acid or other fillers before, you may have areas where collagen has built more than others, creating small nodules or asymmetry. Dr. Emer uses the cannula to push through that scar tissue, almost like a subcision. You can hear it release in the video. That release smooths the contour and lets the new filler distribute evenly.
He also injects toward the base of the shaft to add length. The Alloclae goes into the pubic area and the lower shaft, which can give half an inch to an inch of visible length. The patient in this video already had good length, so the gain was modest, but the principle is the same: releasing scar tissue and adding volume where the shaft meets the body.
Small nodules that remain can be injected with steroid and 5-fluorouracil to shrink them, or simply camouflaged with more filler. Dr. Emer marks asymmetries before he starts and addresses each one during the session.
Downtime and what to expect
The procedure is done in-office with nerve blocks. You feel nothing during the injections. There is minimal bleeding because the cannula is blunt and pushes tissue aside rather than cutting through it. Bruising is light and resolves in one to two weeks.
Dr. Emer asks you to avoid sex and masturbation while you are bruised, typically one to two weeks. He does not use wraps or compression. The Alloclae locks into place on its own, though he sometimes adds a small amount of hyaluronic filler to glue it in and reduce movement.
You can combine this with treatments that improve blood flow and healing: tadalafil (Cialis) or other PDE5 inhibitors, PT-141 peptide, SOURCE:CODE (Dr. Emer's PRP protocol with exosomes, stem cells and peptides), and red light therapy. Those steps are optional but help the tissue integrate the filler faster.
Who this fits
This combination works for men who want more girth and head size, especially if you have had filler before and now have irregularities or nodules. It also fits men who want a more natural feel than hyaluronic acid or PMMA alone, because the Alloclae is softer and the fat cells integrate with your own tissue.
You need patience. The silicone builds over multiple sessions, and rushing the dose creates lumps. The Alloclae gives immediate volume, but the final contour takes weeks as swelling resolves and collagen forms. If you are looking for a single-session result, this is not the plan.
Dr. Emer has seen complications from other practices recently. This is an off-label use of FDA-approved materials, which is legal in the United States, but the technique matters. Go to someone who has done this work repeatedly and knows how to place the product, manage scar tissue and avoid clumping.
Questions about this treatment
How long do the results from Alloclae and silicone last?
Alloclae is permanent: the fat cells and extracellular matrix stay as long as you do not lose significant weight. Silicone is also permanent. Your body builds collagen around the silicone over weeks and months, and that collagen is there for good. Individual results vary based on how much collagen your body produces.
Can this procedure fix lumps from previous filler?
Yes. Dr. Emer uses the cannula to break through scar tissue and release nodules from prior Bellafill, hyaluronic acid or other fillers. The Alloclae then camouflages the irregularities and evens out the contour. Small nodules that remain can be injected with steroid and 5-fluorouracil or simply covered with more filler.
Why not just use hyaluronic acid for the glans?
The glans is spongy tissue. Hyaluronic acid absorbs into that sponge without much visible expansion, so you need large volumes and the result does not last. Silicone stays where it is placed and builds permanent volume as your body forms collagen around it. You get more size for less product.
What are the risks of silicone in the glans?
When injected in microdroplets under two to two and a half cc's per session, studies show no complications. If you inject too much at once or place it too deep, you risk clumping and granuloma formation. Dr. Emer limits the dose and injects superficially to avoid those outcomes. Individual results and risks vary.

Clinical note. Exosome and stem cell products are not FDA approved for these uses. PRP for sexual function and tissue healing is considered experimental. Low-intensity shockwave therapy for erectile function is considered investigational. Fillers used for penile enhancement are used off-label; no filler is FDA approved for this area.
