Girth Enhancement For Uncircumcised Men: HA Plus PMMA With Cannula Technique

How Dr. Jason Emer approaches penile girth enhancement for uncircumcised patients using hyaluronic acid blended with PMMA, creating a plane that accommodates extra skin and builds toward natural, symmetric results.

Why uncircumcised anatomy requires a different approach

The patient in this video is uncircumcised, which changes how filler is placed and how much is needed to achieve a natural result. Extra skin means more surface area to fill, more movement during placement, and a higher risk of bunching if the material is not distributed evenly.

Dr. Emer uses a cannula to create a plane under the skin first, binding the filler to the tissue in a way that reduces irregular settling. This is the foundation: a smooth, even layer that the skin can glide over without creating lumps or asymmetry as it moves.

Building the result in stages

The treatment starts at the top half, working circumferentially around about 270 degrees, then moves down the sides with a needle. The goal is to fill the full circumference, not just the width, so the result reads as natural thickness rather than a top heavy hump.

In the video Dr. Emer explains that trying to fill the entire area at once without enough product leads to irregular results. Instead, he blends what is placed in this session and plans for additional sessions to build the final outcome, especially when the patient has significant extra skin.

How much filler is actually needed

When Dr. Emer pulls the skin back in the video, the amount of area that could be filled becomes visible. For this patient, that could be as much as 100 cc's of filler based on the skin he has. The session shown uses a fraction of that, which is why the result is described as a starting point rather than a final outcome.

More length, more skin, and larger initial size all require more product. There is no universal answer to how much girth one session will add, because every patient starts from a different baseline.

Aftercare and what to expect

Shock wave therapy is used immediately after placement to help circulation, reduce the chance of lumps or bumps, and encourage even settling. The device used in the video is a Z-Wave, and the treatment is described as loud but not painful.

Aftercare includes red light therapy, arnica, bromelain, and in some cases PRP and peptide injections to speed healing. Skin care is also part of the protocol, especially for lighter skin tones, to prevent pigmentation changes.

Dr. Emer is direct about expectations: penile enhancement is never perfectly symmetric from the start, especially for uncircumcised patients. There will be some asymmetry, and more filler than initially expected is almost always required. Candidacy depends on understanding that and planning accordingly.

Every plan starts with a private consultation, in West Hollywood, New York or by video.

Clinical note. PRP for sexual function and tissue healing is considered experimental. Low-intensity shockwave therapy for erectile function is considered investigational. Fillers used for penile enhancement are used off-label; no filler is FDA approved for this area.
