Revision Lipo for Men: Fixing a MAXORIS APEX Body Sculpting That Missed

Dr. Emer shows what it takes to fix body sculpting that left scar tissue, sagging and the wrong shape: VASER, Renuvion, fat grafting and plastic surgery.

Revision body sculpting corrects liposuction that left scar tissue, sagging or the wrong shape. Dr. Emer uses VASER to break through scar tissue, Renuvion to tighten loose skin, fat grafting to restore volume and sometimes plastic surgery to lift sagging tissue. The work is harder than the first surgery because scar tissue blocks access to fat and skin.

What makes revision body sculpting harder than the first surgery

This patient had liposuction before but was left with indentations, scar tissue, sagging chest skin and abs shaped in the wrong place. Dr. Emer had to break through that scar tissue, release the skin and reshape the entire torso.

Scar tissue from the first surgery thickens the tissue under the skin. It blocks access to fat and makes the skin feel firm even when no fat remains. Dr. Emer uses a super wet technique, injecting more tumescent fluid than usual, to lift the skin and soften the scar tissue so he can work through it.

VASER ultrasound breaks apart scar tissue without damaging the fat cells. That matters because this patient needed fat grafting to lift his sagging chest and add volume to his upper buttock. Heat-based devices would destroy the fat and leave nothing to graft.

How Dr. Emer reshaped abs that were placed wrong the first time

The patient's abs were shaped long and not symmetrical to his natural anatomy. Dr. Emer had to sculpt deeper and more precisely to create the six-pack in the right place.

He worked in layers, sculpting deep to define the muscle borders and then sculpting superficially to smooth the skin. The scar tissue required meticulous work to avoid new indentations.

Renuvion tightened the lower abdomen where the skin was loose and indented. The plasma energy shrinks the underside of the skin so it contracts as it heals. Darker skin is thicker, so Dr. Emer used Renuvion more aggressively to get the tightening this patient needed.

Why the chest required plastic surgery

The patient had significant sagging in his lower chest and puffy gland tissue that liposuction alone could not fix. Dr. Emer brought in a plastic surgeon to perform a modified nipple lift and remove the gland.

The surgeon made a semicircular incision along the lower edge of the areola and removed the excess gland tissue. A one-centimeter layer of fat was left under the skin to prevent the areola from contracting as it healed.

Dr. Emer fat grafted the upper chest to add volume and lift. The fat came from the patient's own body, harvested during the VASER portion of the surgery. He did not have much fat because of the scar tissue, but the amount he had was equivalent to about 60 vials of Sculptra in lift.

What the back and buttock sculpting involved

The upper back had minimal liposuction the first time and still had some fat. The lower back had been worked before and was mostly scar tissue and thickened skin.

Dr. Emer used VASER to break apart the scar tissue and contour the back. He removed what fat he could and then used Renuvion to tighten the skin. When no fat comes out during liposuction, the firmness is just thick skin, not fat.

He fat grafted the upper buttock to create a subtle lift. The fat was strained, rinsed with antibiotic solution and loaded into syringes for injection. The goal was a natural lift, not volume.

What recovery from revision body sculpting looks like

Recovery is longer and more uncomfortable than a first surgery because the body is healing through scar tissue. Swelling lasts longer and the skin takes more time to contract.

The super wet technique reduces bruising and swelling compared to working dry, but expect significant swelling for the first few weeks. Compression garments are worn continuously to support the skin as it tightens.

The chest lift adds a healing timeline of its own. The incisions around the areola heal over several weeks, and the patient must avoid lifting or stretching the chest during that time.

Who should consider revision body sculpting

Revision is for men who had liposuction that left them with scar tissue, indentations, sagging skin or a shape that does not match their anatomy. It is not a quick fix. It requires advanced skill and sometimes plastic surgery.

Dr. Emer is direct about what revision can and cannot do. He can improve definition, release scar tissue and tighten skin, but he cannot always make the result perfect. Some scar tissue is too dense to fully correct.

Very few surgeons perform revision body sculpting because of the difficulty. Going to an expert the first time avoids the need for revision. If you are already in that position, choose a surgeon who has the technical skill to work through scar tissue and the honesty to tell you what is realistic.

Questions about this treatment

Can revision body sculpting fix liposuction that left indentations?

Yes, but it depends on the severity. Dr. Emer uses VASER to release scar tissue, Renuvion to tighten skin and fat grafting to fill depressions. Some indentations improve significantly; others improve partially. The outcome is decided in consultation after Dr. Emer examines the scar tissue.

Why does revision body sculpting sometimes require plastic surgery?

Liposuction cannot lift sagging skin or remove excess gland tissue. When the chest or abdomen has significant sagging, a surgical lift is the only option. Dr. Emer combines plastic surgery with VASER, Renuvion and fat grafting to get the best result.

How much fat can be harvested during revision liposuction?

It varies. Scar tissue makes fat harder to harvest, and some patients have very little fat left after the first surgery. This patient had enough for about 60 vials worth of grafting to the chest and buttock, but that is not guaranteed for every revision case.

Is revision body sculpting more painful than the first surgery?

Yes, because the surgeon is working through scar tissue. The super wet technique and Renuvion reduce bruising and swelling, but recovery is longer and more uncomfortable. Compression garments and limited activity are required for several weeks.
