Dr. Jason Emer performs revision body sculpting at Emer Male Clinic when prior surgery leaves scar tissue, lost definition or uneven contours. This patient's plan combined MAXORIS high-definition liposculpture to release scar tissue and rebuild the V-taper, chest sculpting with fat grafting and plasma, internal hernia repair without external incisions, and filler to enhance the abs.
What causes scar tissue and lost definition after body sculpting?
Scar tissue forms when technology is used too aggressively, aftercare is incomplete, or the approach does not match the patient's tissue. This patient was athletic and fit, but in certain lighting and with movement, the skin did not contract properly. He had lost definition in his abs, and the chest area had not been treated well in the other procedures.
The whitish fat Dr. Emer removed during revision showed significant scar tissue mixed with the fat cells. That scar tissue creates irregularities and indentations. Releasing it and smoothing it out requires ultrasound, plasma and the right fluid technique.
How does Dr. Emer approach a revision case?
Dr. Emer uses a super wet technique: overfilling the area with tumescent fluid to hydrate the fat, push scar tissue away and lift the tissue. With tighter, thicker skin and previous surgery, this step is necessary before any sculpting begins.
Ultrasonic liposculpture releases the scar tissue and pushes fat cells aside. The steam heat from the ultrasound shrinks and smooths scar tissue. Renuvion plasma then tightens loose skin and shrivels remaining scar tissue, smoothing out indentations and irregularities. The combination evens out the contour before any fat grafting or enhancement.
What does chest sculpting with fat grafting involve?
Dr. Emer sculpts and defines the chest borders, removes puffy tissue, lifts the chest and makes it more defined. This patient's plan used fat grafting and plasma rather than implants.
The fat is purified and mixed with stem cells and exosomes to help it survive. Dr. Emer places it to create a gladiator chest: defined borders, lifted contour and no excess tissue. Plasma smooths everything out and gives a better overall contour.
How is a hernia repaired during body sculpting?
This patient had a hernia. Dr. Emer repaired it from the inside rather than opening the hernia from the outside and leaving a scar. He tightened the hole and placed back all the fat that came from inside the belly, closing the hole without an external incision.
This approach avoids additional scarring and integrates the repair into the sculpting plan. The hernia is fixed, and the contour is improved in the same session.
What is filler-based ab enhancement?
After the revision liposculpture defined the sides and middle, Dr. Emer used filler to make the abs pop. He placed a small amount of fat into the fascia and muscle to volumize the area slightly, then waited for healing and for the patient to start working out.
He then used a combination of permanent and non-permanent filler to build out the abs. Placement is critical: the filler adheres into the tissue and the fascia of the muscle, creating more definition. Dr. Emer has used this approach for many years, and it looks really good when done well. It is the same combination used in PHALLONOX to build size.
Who is a candidate for revision body sculpting with hernia repair?
This approach fits men who have had prior body sculpting that left scar tissue, lost definition, uneven contours or irregularities. It also fits men who have a hernia that can be repaired internally during the revision.
The patient should be athletic or fit, with realistic expectations about healing and the staged nature of the plan. Ab enhancement with filler is done after initial healing, once the patient has started working out again. Individual results vary, and the full plan is decided in consultation with Dr. Emer.
Questions about this treatment
How long does revision body sculpting with hernia repair take?
The session involves multiple steps: super wet tumescent fluid, ultrasonic liposculpture to release scar tissue, Renuvion plasma to tighten skin and smooth irregularities, chest sculpting with fat grafting, and internal hernia repair. The full procedure is performed in one session. Ab enhancement with filler is done later, after initial healing.
What does scar tissue from prior surgery look like?
Dr. Emer describes the fat removed during revision as whitish, showing significant scar tissue mixed with the fat cells. That scar tissue creates irregularities and indentations. Releasing it requires ultrasound, plasma and the right fluid technique to smooth out the contour.
Can a hernia be repaired without an external incision?
Yes. Dr. Emer repaired this patient's hernia from the inside during the body sculpting session. He tightened the hole and placed back the fat that came from inside the belly, closing the hole without opening the hernia from the outside and leaving a scar.
When is filler used to enhance the abs?
Dr. Emer uses filler after the revision liposculpture has defined the sides and middle, and after initial healing. The patient starts working out, then Dr. Emer places a combination of permanent and non-permanent filler into the tissue and fascia of the muscle to build out the abs and create more definition.
